TRANSFERENCIA
After you have put in your appointment request online you will get an email or text message confirming the appointment. When you come into your appointment you need to bring your:
– Driver’s License
– Medical Marijuana License (if you have it, this makes it easier to find you in the Medical Marijuana User Registry (MMUR)
Before your appointment, you must remove your current physician in the Medical Marijuana Use Registry (MMUR) in order to be successfully transferred to our clinic. If this step is not completed, we will be unable to enter your new recommendations.

When you arrive, you will sign-in at the front desk and receive a clipboard with a short questionnaire to complete. Once completed you will be checked in by our front desk where they will collect your intake forms, valid state-issued ID, and payment.
